EACH GUEST MUST READ ALL TERMS AND CONDITIONS OF ATTENDING FIELD TO TABLE IN FULL. ATTENDANCE CONSTITUTES AN AGREEMENT OF THE BELOW TERMS & CONDITIONS
The safety and health of our guests is our top priority. If you are feeling sick, please stay home.
By purchasing a reservation and attending Field to Table, you & all guests in your party agree to the following:
AN INHERENT RISK OF EXPOSURE TO COVID-19 EXISTS IN ANY PUBLIC PLACE WHERE PEOPLE ARE PRESENT. COVID-19 IS AN EXTREMELY CONTAGIOUS DISEASE THAT CAN LEAD TO SEVERE ILLNESS AND DEATH. ACCORDING TO THE CENTERS FOR DISEASE CONTROL AND PREVENTION, SENIOR CITIZENS AND THOSE WITH UNDERLYING MEDICAL CONDITIONS ARE ESPECIALLY VULNERABLE. BY ENTERING THE STADIUM AND STADIUM GROUNDS, HOLDER VOLUNTARILY ASSUMES ALL RISKS RELATED TO EXPOSURE TO COVID-19.
Sound Excursions, LLC meets all guidance and requirements set out by the State of Washington, King County, the City of Seattle, and the Center of Disease Control and Prevention (CDC). Sound Excursions, LLC and Field To Table prides itself in prioritizing safety for the customer and the staff for the safest possible outdoor dining events.
All attendees age 12 and over will be required to provide proof of COVID-19 vaccination for entry at Lumen Field. A photo of your vaccine card works as proof. In lieu of vaccination verification, fans may also present a negative COVID-19 test result taken within 72 hours prior to the event. Documentation must be from a testing agent such as a laboratory, health care provider, or pharmacy. Self-administered rapid tests will not be accepted.
Groups of 2-10 are permitted.
All guests must sanitize hands upon arrival. Wash your hands frequently using the on-site restrooms and the hand sanitizer stations between hand washing.
Masks will be required at all times when not seated at your table. No exceptions will be made.
All surfaces will be cleaned and disinfected preceding reservations.
Dining areas are separated in open-air areas to ensure a safe distance between diners at all times.
LUMEN FIELD WAIVER & RELEASE FROM LIABILITY
PLEASE READ THE FOLLOWING CAREFULLY:
In return for being allowed to participate in the activities on Lumen Field, I release and agree not to sue Football Northwest LLC, d/b/a Seattle Seahawks, First & Goal Inc, Sound Excursions LLC, and any of the foregoing entities’, successors, assigns or legal representatives, and any owners (including without limitation parents, affiliates, members, partners or share holder, whether direct or indirect), subsidiaries, employees, sub-contractors, agents, family members, estate or heirs of any of the foregoing (the “Released Parties”) from any and all present and future claims resulting from ordinary negligence on the part of the parties listed above for property damage, personal injury, or wrongful death, arising as a result of my participation in the Lumen Field activities, wherever, whenever of however the same may occur. I hereby represent that I have authority to and do voluntarily waive any and all claims resulting from ordinary negligence, both present and future, that may be made by me, my family, estate, heirs or assigns. I am voluntarily participating in the Lumen Field activities with knowledge of the danger involved and agree to accept all risk of participation. I also agree to indemnify and hold harmless the Released Parties for all claims arising out of my participation in the Lumen Field activities and all related activities. I agree to let the Released Parties use my name and likeness free of charge in any manner for any purpose. I understand that this document is intended to be as broad and inclusive as permitted by the laws of the State of Washington and agree that if any portion of this agreement is invalid, the remainder will continue in full legal force and effect. I further agree that any legal proceeding related to this waiver would take place in Washington State.
I have read this form and fully understand that by purchasing and attending Field to Table, I am giving up legal rights and/or remedies that may be available to me for the ordinary negligence of the Released Parties.
Guest Acknowledgement and Statement of Compliance
The novel coronavirus (“COVID-19”) is a highly infectious, life-threatening disease declared by the World Health Organization to be a global pandemic. COVID-19’s highly infectious nature means that contact with others, or contact with surfaces that have been exposed to the COVID-19 virus, may lead to infection. Additionally, some individuals who contract COVID-19 are asymptomatic. Because of COVID-19’s highly contagious, and sometimes asymptomatic, nature, it is still very difficult to control the spread of COVID-19 despite the imposition and execution of numerous safety precaution.
Agreement to Comply with Field to Table Rules
Aware of the nature of COVID-19, you, those joining you at Field to Table, and any persons for whom you have purchased a ticket or dining experience for at Field to Table and who visit Field to Table (collectively, the “Guests”) are voluntarily entering Lumen Field to dine with or visit Field to Table. You and the other Guests understand that Field to Table has put in place safety rules and precautions in order to mitigate the spread of COVID-19, which rules and precautions may be updated at any time. By visiting Field to Table you and the other Guests agree to comply with Field to Table’s rules and precautions which include, but are not limited to, practicing social distancing, wearing an appropriate face covering at all times that you are not eating, only removing your face covering while seated at your table and dining, limiting group sizes to six people, limiting groups to people from no more than two households, and sanitizing your hands upon arrival and as necessary throughout your visit at Field to Table. By visiting Field to Table you and the other Guests confirm that you have read, understand, and will comply with Field to Table’s rules.
Acknowledgment and Assumption of Risk
COVID-19
To comply with Centers for Disease Control and Prevention (“CDC”) guidelines, state and local laws and guidelines, and public health best practices during the COVID-19 outbreak and aftermath, we respectfully ask that by attending Field to Table at Lumen Field, you acknowledge and agree that you and the other Guests are fully aware of the symptoms of COVID-19 and confirm that none of the Guests are experiencing any such symptoms, nor do any of the Guests have reason to suspect that they have been exposed to COVID-19 within the past 14 days. Additionally, you fully understand that Sound Excursions, LLC and Field to Table cannot guarantee that the facility, its guests, its contractor, or its employees are free of the COVID-19 virus.
We are asking that our valued guests and visitors, including the Guests, follow all recommended CDC and Washington State Department of Health guidelines relating to COVID-19. We are all responsible for practicing the preventative steps recommended by the CDC and the Washington State Department of Health and your thoughtful attention to these guidelines is needed to help prevent the spread of COVID-19.
In connection with your visit to Field to Table at Lumen Field, we are requiring the acknowledgement and consent to the following terms:
I, on my own behalf and on behalf of the other Guests, acknowledge that the Guests fully understand that COVID-19 is highly contagious, and that although Field to Table is taking reasonable efforts to mitigate contamination risks, all risk of exposure cannot be fully eliminated. As such, the Guests are assuming the inherent risk of exposure to COVID-19 by visiting and/or dining with us or any other public place offering dining services. In consideration of visiting Field to Table at Lumen Field, the Guests hereby waive and release the Lumen Fields, its owners and managers, Sound Excursions, LLC, Field to Table, and each of their respective affiliates, subsidiaries and employees from any and all claims, liabilities, damages, actions or costs directly or indirectly arising from or in connection with potential or actual exposure to COVID-19 at Field to Table at Lumen Field (or at any medical facility or testing center visited subsequently) including, but not limited to, any illness, injury, loss of work, loss of wages, trauma, emotional distress, medical expenses or death suffered in connection therewith.
Pop-Up Nature of the Outdoor Dining Experience
Further, I, on my own behalf and on behalf of the other Guests, acknowledge that myself and the other Guests fully understand that Farm to Table is a pop-up dining experience created to offer a dining experience under the state’s current COVID-19-related laws and protocol, I on behalf of myself and on behalf of the other Guests, hereby release, acquit, and hold harmless Sound Excursions, LLC, Field to Table, First & Goal Inc. and all affiliates from any and all causes of action related to attendance at the dining experience related to the pop-up nature of the outdoor dining experience, including any claims related to tents, heating, COVID-19 prevention measures, electrical work, and other foreseeable . I, on my own behalf and on behalf of the other Guests, acknowledge that the Guests fully understand that the pop-up nature of the dining experience may pose certain risks, and that although Field to Table is taking reasonable effects to mitigate these risks, all risks cannot be fully eliminated. In consideration of visiting Field to Table, the Guests hereby waive and release the property owner, property manager, Sound Excursions, LLC, Field to Table, and each of their respective affiliates, subsidiaries and employees from any and all claims, liabilities, damages, actions or costs directly or indirectly arising from or in connection with the pop-up nature of the dining experience including, but not limited to, any illness, injury, loss of work, loss of wages, trauma, emotional distress, medical expenses or death suffered in connection therewith.
By visiting Field to Table at Lumen Field I acknowledge, on my behalf and on behalf of the other Guests, that I have informed all Guests of these terms, that the Guests fully understand the waiver set forth above, and that each of the Guests fully accepts all risks posed by the dining experience and risk of exposure to COVID-19 and the pop-up nature of the Field to Table in connection with visiting Field to Table at Lumen Field.

I, and on behalf of my Guests that I bring and/or purchase tickets for, permits Field to Table, Sound Excursions, LLC, First & Goal Inc., and their respective affiliates and assigns to use and exploit the names, likeness, image, voice and other indicial identity of any of us at the Food to Table outdoor dining experience for the purposes of (a) providing information to the general public about the dining experience and events at the Stadium Complex, (b) advertising and promoting the Food to Table outdoor dining experience, (c) if applicable, selling, soliciting and promoting the purchase of tickets for the outdoor dining experience or any merchandise, goods, or services associated with the outdoor dining experience, and (d) general promotion of the Stadium Complex or the Food to Table outdoor dining experience.
